Treatment
  1. In Vitro Fertilisation - I.V.F (Popularly known as Test-Tube Baby) is a panacea for almost all the causes of infertility.

IVF is done in following conditions : Bilateral tubal block, endometriosis
Severe Pelvic Inflammatory Disease (PID), mild to moderate degree of male infertility.
    Unexplained infertility.

Investigations to be done before IVF : Day1- Endometrial Biopsy(EB)
    Day2- Serum Hormones
    Day7- Hysteroscopy
    Husband's seminal fluid examination (HSF)
Fee : 40,000/ cycle including drugs
    Success Rate 25% per cycle

  2. Intra Cytoplasmic Sperm Injection- ICSI : This method is useful for patients having very low sperm in the male partner. This also used in patient with no sperms by retrieving sperm from their tests. it is also helpful in couples who had fertilizations failure in previous IVF.

This process is almost same as that of IVF.

    Cost Rs. 50,000-60,000
    Success rate- 25% per cycle

  3. Intra Uterine Insemination (IUI) : (Combined with semen wash) This is a low cost treatment of difficult cases of infertility where at least one tube should be patent and sperm count should be more than 10 million with 30% motility.
Patient has to come on Day9 after ovarian stimulation with Clomiphene and HMG when a scan is done and IUI is done on D11/12 in most of the cases.

Cost  Rs. 2,000 to Rs. 4,000/Cycle (Success Rate 5%/ Cycle)

  4. Donor embryo/Oocyte : This is a modification of IVF used in elderly women and when due to ovarian failure a woman has no oocytes. In these cases presented by amenorrhoea and high FSH in recipient woman, oocytes are aspirated from another woman (donor) and fertilized with recipients eggs. The embryos then formed are replaced in recipient's uterus. This is also suitable treatment for woman of peri menopausal age group who want to conceive.

Cost of treatment Rs. 50,000/Cycle (Donor has to be arranged by the recipient.)

  5. Donor insemination (AID-Artificial Insemination with Donor semen) : In almost 50% of the infertile couples, the cause lies in male partner. The treatment of male infertility is restricted to correction of hypogonadaotrophic hypogonadism with HMG/HCG. Evidence is lacking requiring the benefits of VEA, varicocoel ligation and clomiphene. Time treats many of these males, but after some time, the options are ICSI, donor insemination and adoption.

AID has got very good results (10-15%per cycle) when it is done employing IUI. Cryopreserved semen is used which has no risk of sexually transmitted diseases (STD ), there is ready availability of semen from donor of different blood groups and option to match donor having similar characteristics such as height, complexion and built as that of husband.

    Fee Rs. 2100/-cycle

  6. Semen banking : is used for donor insemination. Homologous Semen can also be banked in cases of IUI when husband can have ejaculatory or erectile dysfunction at the time of IUI due to anxiety . It is also recommended before vasectomy, operation for hydrocoel, orchidectomy for testicular tumor or before chemotherapy when future childbearing is required.

    Fees of Banking Rs. 2,000/year

  7. Embryo Banking : Embryo can be frozen in the cases of IVF and also when donor oocytes is being used and these can be used in subsequent cycles.

Diseases
PCOD- It is a condition in which imbalance in hormones leads to inadequate follicular growth thereby also producing menstrual irregularity and hairiness. Treatment is by clomiphene, hormones and ovarian drilling.

Endometriosis- In this diseases the lining of uterus is abnormally present in other places. This presents as pain during menstruation, pain during intercourse and infertility. Drugs are use sometimes to suppress the ovarian activities. In advanced cases surgery is done to break the adhesions and rarely to remove the ovary. IUI and IVF is useful in many of these women.

Premature Ovarian Failure- Loss of all the oocytes (eggs) before the age of 35-40 years of age is called P.O.F. This causes absence of periods and is diagnosed by high FSH in blood. The treatment of infertility is by IVF using oocytes from another women.

Azoospermia- It is complete absence of sperm in semen. The treatment option are Intra cytoplasmic sperm Injection (ICSI-MESA/TESA), vaso-epididymal anastomosis (VEA) or donor insemenation (AID).

Pelvic infection- Various infections like sexually transmitted diseases and tuberculosis can adversely affect fertility. Treatment is by antibiotics, microsurgery or ART.
